Death End is a mechanic that gives the player an instant game over for taking specific incorrect actions. Many of them result the death of Shina Ninomiya, Arata Mizunashi, Mai Toyama, or someone close to them, and they often occur outside of the Death end re;Quest series' standard combat gameplay. Death end re;Quest features more than two dozen Death Ends, while Death end re;Quest 2 features a smaller, but still notable, number of instant death scenarios. Death end re;Quest: Code Z is set to feature 104 Death Ends, the most out of any prior entry in the series. The game also features "Double Death Ends", where through specific interactions with certain characters during a certain point, the game shows Sayaka and another character being killed or dying through a unique CG.

Players can gain items on the Episode Chart by seeking out these non-standard Game Overs.

It is implied in the final chapter of Death end re;Quest that all Death Ends are canon, and are undone by the God of Death.

Death end re;Quest: Code Z[]

Death end re;Quest: Code Z has 104 different death ends. Each one depending on how Sayaka dies. Every time Sayaka dies, you will obtain a death end skill tree point to use while in the Cafe. Each of the skills on the skill tree will give Sayaka an edge over her enemies by increasing health, defence, attack, reducing damage from different enemy types, and certain bonuses when entering the vanquish form.

Rach enemy type in the game has a unique death end. Each resulting in a detailed description of Sayaka’s death as well as showing one of 8 CG artworks depicting Sayaka in certain death scenarios.

Other Death End CGs are obtainable through dying at certain times, such as while being accompanied by a supporting character or “VIP” such as Shina, or Lily. Dying in these moments to a normal enemy type will result in a description as well as an accompanying death CG of Sayaka and the accompanying character. This is true for strong enemies as well such as ‘the glitch girls’. These death ends are what were called “Double Death Ends” before release.

Death end re;Quest 3[]

Due to the game currently being unreleased, most if not all of the Death Ends are currently unknown, but the game will reportedly feature the ability to "predict" Death Ends.
